Hopewell Member Facility and Equipment Request Form

Hopewell Baptist Church encourages use of its facilities by church members who are at least (21) years of age, small group fellowships, and Sunday School classes. However, because scheduling of facilities must be done to accommodate different projects or programs, and because the facility and its equipment must be protected by a responsible member, this form is necessary.
 
Please complete the form to check the date and reserve facilities and equipment for your activity.

NOTE: The person filling this form out must be a member of Hopewell Baptist Church and at least (21) years of age and will be responsible for providing their own supplies such as decorations, disposable paperware (plates, cups, etc) Disposable plasticware (forks, spoons, etc) as well as tablecloths.

Setup for this activity and restoration of the facility to their original condition or Sunday use, whichever is appropriate, is the responsibility of the one filling out this form.

  • If this request is for a wedding, you will be given, or mailed, copies of the Church Wedding Policies to sign.
  • If requesting the Sanctuary, please contact the church office.
NOTE: THE CUSTODIAN IS NOT RESPONSIBLE FOR CLEANUP. 
THE FACILITY MUST BE ORGANIZED AND CLEANED TO THE ORIGINAL STATE.
